Location: Carmichael, CA

Start Date: 07-07-2026

End Date: 10-06-2026

Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM, with rotating weekends (at least one full weekend per month)

Hours per Week: 40

Candidate Pay Rate: $82 per hour

Roles & Responsibilities

Conduct initial assessments and ongoing care coordination for patients
Perform utilization review using Interqual criteria
Develop and implement discharge plans across all hospital areas
Collaborate with multidisciplinary teams to ensure effective patient transitions
Manage cases involving various payers, uninsured populations, and homeless individuals
Document all activities in Cerner EMR system

Qualifications

Active RN license in California
BLS certification required
Prior recent experience with Dignity Health or CommonSpirit facilities (preferred)
Case Management or Utilization Review experience a plus
Willingness to learn Interqual (training provided)

Knowledge & Skills

Strong collaboration and teamwork abilities
Excellent communication skills
Ability to reprioritize throughout the day
Knowledge of discharge planning and utilization review processes
Comfortable working with diverse patient populations
