Contract Type: Contract

Shift Type: Monday-Friday, 8:30 AM - 5:00 PM, with rotating Saturday once a month

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Assess and evaluate patients' physical therapy needs in their home environment
Develop and implement individualized treatment plans to improve mobility, strength, and function
Educate patients and families on therapeutic exercises and home safety
Collaborate with physicians and other healthcare professionals to ensure coordinated care
Document patient progress and adjust treatment plans as needed
Supervise home health aides and participate in case conferences

Qualifications

Minimum of 2 years of experience as a Physical Therapist in acute, home care, or rehab settings
Active Physical Therapist license in California
Current BLS certification
Excellent communication and organizational skills
Ability to work independently and manage a caseload across multiple locations

Knowledge & Skills

Strong clinical assessment and treatment planning skills
Knowledge of Medicare, Medi-Cal, and Title 22 regulations
Proficient in documentation and electronic health records
Compassionate and patient-centered approach
Reliable transportation and ability to travel within Yolo County
