Candidate Pay Rate: $59 per hour

Roles & Responsibilities

Conduct comprehensive home health assessments to develop tailored care plans
Implement therapeutic exercises and interventions to improve patient mobility
Collaborate with physicians, nurses, and other healthcare professionals for coordinated care
Educate patients and families on injury prevention and home exercise programs
Document patient progress accurately and maintain compliance with regulations

Qualifications

Active California Physical Therapist license
BLS certification from the American Heart Association
Minimum 1 year of home health experience preferred
Strong communication and organizational skills
Ability to travel locally up to 25% of the time

Knowledge & Skills

In-depth knowledge of home health regulations and documentation standards
Proficiency with electronic medical records (EMR) systems
Excellent interpersonal and patient education skills
Ability to work independently and manage a caseload efficiently
