Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Conduct comprehensive home health assessments including start of care, recertifications, and resumption of care
Develop and implement individualized physical therapy plans to promote patient independence
Educate patients and families on therapeutic exercises and safety strategies
Collaborate with interdisciplinary team members to ensure coordinated care
Document patient progress accurately and timely in accordance with regulatory standards

Qualifications

Active North Dakota Physical Therapy License
Current BLS certification (American Heart Association)
Valid Driver's License and personal auto insurance
Home Health experience and OASIS training preferred
Comfortable with daily travel to patient homes within a 50-mile radius

Knowledge & Skills

Proficient in OASIS documentation and home health regulations
Excellent clinical reasoning and problem-solving abilities
Strong communication and interpersonal skills for patient and family education
Ability to manage a caseload of 5-6 visits per day including admissions
