Contract Type: Travel Contract

Shift Type: Rotating schedule including weekends, 8:00 AM - 5:00 PM, 40 hours per week

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Provide comprehensive physical therapy assessments and treatments to patients in their homes
Develop and implement individualized care plans tailored to each patient's goals
Educate patients and families on therapeutic exercises, home safety, and fall prevention
Perform start of care, recertifications, and evaluations as part of daily caseload
Collaborate with interdisciplinary team to ensure coordinated, high-quality care
Document patient progress accurately and timely using provided tablet

Qualifications

Current BLS certification required
Active Physical Therapy license in Kentucky or compact license eligible
Prefer experienced therapists; new grads considered with strong clinical support
Home health experience a plus but not required
Ability to work independently and manage a caseload across multiple locations

Knowledge & Skills

Home health regulations and compliance standards
Patient assessment and care planning for diverse diagnoses (CHF, ortho, pneumonia)
Therapeutic exercise and manual therapy techniques
Electronic health records and mobile documentation tools
Time management and autonomous decision-making in the field
