Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Conduct start of care, recertifications, and resumption of care visits for home health patients
Perform 6.5 points per day, including regular visits and admissions
Collaborate with clinical coordinator and director for support in the field
Provide patient teaching and safety evaluations in hospice situations as needed

Qualifications

Active North Dakota Physical Therapy License
Current BLS certification
Valid Driver's License and Auto Insurance
Home Health experience preferred
OASIS training preferred
Comfortable with a 1-2 week orientation period

Knowledge & Skills

Proficient in OASIS documentation
Strong patient assessment and care planning skills
Excellent time management for point-based productivity
Ability to work independently in a home health setting
