Roles & Responsibilities

• Provide compassionate direct patient care and assistance with activities of daily living (ADLs)
• Take and record vital signs accurately and report changes to nursing staff
• Assist with safe transfers, mobility, and basic patient comfort measures
• Document care and communicate clearly with the care team for continuity
• Support infection control practices and maintain a safe environment
• Work collaboratively with nurses and rehab staff to meet patient goals

Qualifications

• Current LNA certification (T-LNA / Licensed Nursing Assistant) — valid for VT/NH assignments
• Proven experience providing direct patient care in long-term care or rehab settings preferred
• Reliable transportation required for this assignment (local travel expected)
• Availability to work the evening 3:00 PM - 11:00 PM shift and every-other-weekend coverage
• Ability to pass facility onboarding requirements and background checks

Education

• High school diploma or GED preferred
• Current LNA/T-LNA certification required

Knowledge & Skills

• Strong patient care skills including ADLs and basic nursing tasks
• Competent in taking/recording vital signs and recognizing changes
• Clear documentation and communication skills with interdisciplinary teams
• Infection control and safety-minded practices
• Time management and ability to prioritize in a busy rehab setting
• Team player with empathy and respect for residents
