Roles & Responsibilities

• Evaluate patient functional status and develop individualized treatment plans that promote independence
• Deliver evidence-based therapeutic interventions, including manual therapy and therapeutic exercise
• Document patient progress clearly and promptly in the medical record to ensure continuity of care
• Collaborate with nursing, physicians, and rehabilitation team members to coordinate care goals
• Educate patients and families on home exercise programs and safe mobility techniques
• Monitor outcomes and adjust treatment plans to maximize patient progress and safety
• Participate in team huddles and discharge planning to support smooth transitions

Qualifications

• Current Physical Therapist license in New Jersey or eligibility to obtain one quickly (licensure assistance available)
• Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy preferred
• Basic Life Support (BLS) / CPR certification required or obtainable upon hire
• Comfortable with treating a diverse adult patient population in a post-acute/skilled setting
• Strong clinical reasoning, excellent communication skills, and a collaborative mindset
• Travel or contract experience a plus but not required

Education

• Doctor of Physical Therapy (DPT) preferred
• Master’s in Physical Therapy accepted

Knowledge & Skills

• Skilled in patient assessment, gait training, transfers, and balance interventions
• Proficient with documentation in electronic medical records and outcome measures
• Knowledge of therapeutic modalities and manual therapy techniques
• Ability to develop safe, practical home exercise programs and caregiver training
• Strong interdisciplinary teamwork, time management, and organizational skills
• Infection control practices and safe handling of mobility equipment
