Roles & Responsibilities

• Provide skilled physical therapy evaluations and develop individualized treatment plans
• Deliver hands-on interventions and therapeutic exercises to restore mobility and function
• Lead gait training, balance activities, and functional mobility programs to improve independence
• Monitor patient progress, adjust plans of care, and document outcomes clearly and promptly
• Collaborate with nurses, case managers, and interdisciplinary team members to coordinate patient-centered care
• Educate patients and families on home exercises, safe mobility techniques, and discharge planning
• Advocate for patient needs and contribute to a positive, healing environment

Qualifications

• Active Physical Therapist license in the state of practice (or eligibility to obtain one quickly)
• Current Basic Life Support (BLS) certification
• Experience in inpatient rehabilitation, skilled nursing, or post-acute care environments preferred
• Strong clinical reasoning, documentation skills, and patient-focused communication
• Ability to work Monday through Friday on the listed day shift

Education

• Doctor of Physical Therapy (DPT) or Master's in Physical Therapy preferred; equivalent clinical experience considered

Knowledge & Skills

• Skilled in therapeutic exercise prescription and progression
• Proficient with gait training, assistive device recommendations, and balance interventions
• Knowledgeable in safety-focused transfers and fall prevention strategies
• Comfortable with electronic medical records and clear, timely documentation
• Excellent interpersonal skills for educating patients and collaborating with care teams
