Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents at all stages of need
• Administer medications and treatments accurately and safely
• Monitor and document vital signs, patient status changes, and responses to interventions
• Maintain clear, timely charting to ensure continuity of care and regulatory compliance
• Collaborate with nurses, CNAs, therapists, and leadership to create individualized care plans
• Educate residents and families about care plans, medications, and health promotion
• Follow infection control practices and facility policies to maintain a safe environment
• Participate in quality improvement initiatives and team huddles to enhance resident outcomes

Qualifications

• Active Licensed Practical Nurse (LPN) license; New Mexico licensure preferred or required for practice
• Current CPR/BLS certification (or willingness to obtain prior to start)
• Previous LPN experience in long-term care or skilled nursing preferred
• Ability to work rotating night shifts and local every-other-week rotations
• Strong clinical judgment, dependable attendance, and a team-first attitude
• Pass background checks and meet facility credentialing requirements

Education

• Practical Nursing diploma or equivalent (completed LPN program)

Knowledge & Skills

• Medication administration and safe documentation practices
• Resident assessment and recognition of acute changes in condition
• Familiarity with charting and electronic medical records
• Basic wound care, ADL assistance, and restorative nursing principles
• Excellent communication and teamwork skills with residents, families, and staff
• Time management and the ability to prioritize in a fast-paced setting
