Roles & Responsibilities

Provide exceptional physical therapy care across four rural counties
Conduct comprehensive Oasis Assessments for accurate patient evaluations
Manage Start of Care cases with compassion and expertise
Help elderly patients recover from falls and fractures
Document visits efficiently using modern tablet technology
Build meaningful relationships with patients in their home environments

Qualifications

Current Physical Therapist license in Indiana
Experience with Oasis Assessments and Start of Care documentation
Strong clinical skills in geriatric rehabilitation
Ability to work independently in rural settings
Reliable transportation for multi-county coverage
