Roles & Responsibilities

Deliver personalized physical therapy services in patients' homes across Yolo County
Assess and evaluate each patient's therapeutic needs and create individualized care plans
Guide patients and families through exercises and the safe use of therapeutic equipment
Monitor progress and communicate updates to physicians while documenting outcomes
Collaborate with home health aides and coordinate additional support services as needed
Educate caregivers on therapy goals and techniques to maximize patient independence

Qualifications

Minimum 2 years of physical therapy experience in acute, home care, or rehab settings
Current BLS certification required
Strong communication and organizational skills
Ability to work independently while traveling between patient homes
Valid driver's license and reliable transportation

Knowledge & Skills

Home health physical therapy assessment and treatment
Patient mobility, gait, and range-of-motion evaluation
Care plan development and goal tracking
Family and caregiver education techniques
Compliance with Medicare, Medi-Cal, and Title 22 regulations
