Roles & Responsibilities

Deliver personalized physical therapy to patients recovering from falls and fractures
Complete Oasis Assessments and Start of Care evaluations that guide treatment plans
Visit 4-6 patients daily across four beautiful rural counties
Collaborate with an interdisciplinary team to help patients regain independence
Document visits efficiently using provided tablets for seamless care coordination

Qualifications

Current Physical Therapist license in Indiana
Experience with Oasis Assessments and Start of Care documentation
Comfortable providing care in both rural and small-town settings
Strong communication and time-management skills
