Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents following facility policies
Administer medications and treatments accurately and safely
Document resident status, care provided, and changes in condition clearly and promptly
Collaborate with the care team to develop and follow individualized care plans
Monitor residents for changes and escalate concerns to the RN or provider as needed

Qualifications

Active LPN license in New York (required)
Completion of an accredited practical nursing program
Current BLS/CPR certification or willingness to obtain upon hire
Ability to pass background checks and meet facility onboarding requirements
Strong communication skills and a caring, patient-centered approach

Education

Graduation from an accredited practical nursing program; LPN/LVN certificate or diploma

Knowledge & Skills

Clinical skills in medication administration and basic nursing procedures
Familiarity with documentation standards and electronic charting preferred
Ability to observe and report changes in resident condition
Teamwork and effective communication with interdisciplinary staff
Time management and prioritization in a busy care setting
