Roles & Responsibilities

Provide compassionate direct patient care and assist with activities of daily living
Administer medications and treatments according to physician orders and facility policy
Monitor and document vital signs, patient status, and changes in condition
Follow individualized care plans and report significant changes to the RN or provider
Collaborate with interdisciplinary team members to deliver coordinated care
Maintain accurate and timely clinical documentation and charting
Promote a safe, clean, and therapeutic environment for residents
Participate in shift handoffs and communicate critical information clearly

Qualifications

Active LPN license in Pennsylvania required
Current BLS/CPR certification or willingness to obtain upon hire
Comfortable working with adult and geriatric populations in a long-term care setting
Strong communication skills and a patient-centered approach
Reliable, punctual, and able to follow facility policies and procedures

Education

Graduate of an accredited Practical Nursing program or equivalent certification

Knowledge & Skills

Solid knowledge of medication administration and documentation
Ability to perform basic nursing procedures and ADL assistance
Good assessment skills and recognition of changes in patient condition
Teamwork, time management, and the ability to prioritize tasks
Familiarity with electronic health records is a plus
