Roles & Responsibilities

Provide compassionate bedside care to residents, focusing on safety and dignity
Administer medications and treatments accurately and document thoroughly
Monitor patient status and communicate changes promptly to the care team
Assist with daily living activities and promote independence whenever possible
Collaborate with nurses, aides and interdisciplinary staff to deliver coordinated care
Maintain accurate charting and follow facility policies and infection control protocols

Qualifications

Current, active LPN license (Pennsylvania) required
Completion of an accredited practical nursing program
Basic Life Support (BLS/CPR) certification preferred or available upon hire
Comfortable with medication administration, vital signs and patient assessments
Valid TB screening and up-to-date immunizations as required by facility
Background check and drug screen clearance as part of onboarding

Education

Completion of an accredited LPN (practical nursing) program
High school diploma or equivalent

Knowledge & Skills

Strong clinical assessment and basic nursing skills
Clear, professional communication with patients and team members
Accurate medication administration and documentation practices
Familiarity with infection control and resident safety standards
Teamwork mindset and the ability to work in a fast-paced long term care setting
Basic electronic medical record (EMR) navigation and charting skills
