Roles & Responsibilities

Provide compassionate bedside care and support for residents and patients
Administer medications and follow prescribed treatment plans carefully
Monitor vital signs and report changes in condition promptly
Document care accurately and maintain clear electronic and paper records
Collaborate with nurses, therapists, and facility staff to deliver coordinated care
Assist with admissions, discharges, and safe patient transfers

Qualifications

Current, active LPN license (eligible to practice in Pennsylvania)
Basic Life Support (BLS) / CPR certification (current)
Comfortable with medication administration and basic clinical procedures
Strong communication skills and a team-oriented attitude
Prior experience in long-term care or skilled nursing preferred

Education

LPN diploma or completion of an accredited practical nursing program
High school diploma or equivalent

Knowledge & Skills

Patient assessment and basic clinical monitoring
Medication administration and documentation
Infection control and safety protocols
Electronic health record (EHR) charting
Effective communication with patients, families, and care teams
Time management and prioritization in a busy care setting
