Roles & Responsibilities

Provide compassionate, patient-centered nursing care as a Licensed Practical Nurse
Administer medications and treatments safely and accurately
Monitor patient conditions and communicate changes to the care team promptly
Document care thoroughly in the patient record to support continuity
Assist with activities of daily living and promote patient dignity and independence
Collaborate with nurses and other clinical staff to deliver high-quality care

Qualifications

Active NY Licensed Practical Nurse (LPN) license required
Completion of an accredited LPN/LVN program
Current CPR/BLS certification (or willing to obtain)
Ability to pass background and health screenings as required by facility
Strong communication skills, reliability, and a team-oriented attitude

Education

Graduate of an accredited LPN/LVN program (diploma or certificate) and high school diploma or GED

Knowledge & Skills

Medication administration and basic IV/PO knowledge
Basic wound care and vital sign assessment
Comfortable with electronic charting and clinical documentation
Strong patient observation and reporting skills
Team collaboration and time management in a clinical setting
