Roles & Responsibilities

Provide compassionate, patient-centered nursing care within scope of LPN practice
Administer medications and treatments accurately and document thoroughly
Monitor and record patient vital signs, reporting changes to RN or provider promptly
Collaborate with the interdisciplinary team to coordinate care plans
Assist residents with activities of daily living while promoting dignity and independence
Maintain clear, professional documentation in the patient record
Follow infection control policies and uphold a safe, clean environment for patients

Qualifications

Active LPN license in Pennsylvania (required)
Graduation from an accredited Practical Nursing program
Basic Life Support (BLS/CPR) certification preferred or willing to obtain
Comfortable with medication administration and basic patient assessments
Strong communication, teamwork, and time-management skills
Positive attitude, reliability, and a commitment to quality patient care

Education

Completion of an accredited Practical Nursing (LPN) program and active LPN licensure in Pennsylvania

Knowledge & Skills

Medication administration and documentation best practices
Patient assessment and vital signs monitoring
Basic wound care and infection control procedures
Electronic medical record/charting familiarity
Safe patient handling and mobility assistance
Effective communication with patients, families, and care teams
Time management and prioritization in a busy clinical environment
Professionalism, confidentiality, and patient advocacy
