Roles & Responsibilities

Provide compassionate direct patient care and bedside support
Administer medications and treatments safely and accurately
Assess patient conditions and report changes promptly to the care team
Document care, observations, and communications in the medical record
Collaborate with nurses, therapists, and providers to develop care plans
Support patients and families with clear, respectful communication
Assist with admissions, transfers, and safe patient handoffs

Qualifications

Active Licensed Practical Nurse (LPN) license — Pennsylvania license required or eligible
Current BLS/CPR certification (required)
Comfortable working in a fast-paced long-term care or skilled nursing setting
Strong communication skills and ability to work well within a team
Ability to pass background check and drug screening as required

Education

Completion of an accredited LPN/Vocational Nursing program

Knowledge & Skills

Medication administration and documentation best practices
Patient assessment, vital signs, and basic wound care
Infection control and safe patient handling techniques
Electronic medical record (EMR) charting and documentation
Time management, prioritization, and effective clinical judgment
