Roles & Responsibilities

Provide compassionate bedside nursing care to residents, ensuring dignity and respect
Administer medications and treatments accurately and document in the medical record
Monitor residents vital signs and report changes in condition promptly
Collaborate with RNs, support staff and interdisciplinary team to implement care plans
Support activities of daily living and assist residents with mobility and hygiene needs
Communicate effectively with residents and families to provide updates and education
Participate in quality improvement initiatives and maintain regulatory compliance

Qualifications

Active LPN license and eligibility to practice in Pennsylvania
Completion of an accredited practical nursing program
Current BLS (Basic Life Support) certification or willingness to obtain
Comfortable with medication administration, basic wound care and resident assessments
Strong communication skills and ability to work as part of a multidisciplinary team
Prior experience in long-term care or skilled nursing preferred

Education

Graduation from an accredited practical nursing program (LPN diploma or certificate)

Knowledge & Skills

Medication administration and documentation
Basic wound care and treatment monitoring
Vital signs assessment and basic clinical observations
Electronic medical record/charting familiarity and accuracy
Patient-centered communication and family engagement
Time management, prioritization and teamwork in a busy clinical setting
