Roles & Responsibilities

Provide compassionate bedside care and perform nursing tasks that keep residents safe and comfortable
Administer medications and treatments accurately while following facility protocols
Monitor patient condition and communicate changes promptly to the care team
Document care activities thoroughly and maintain clear, timely records
Collaborate with nurses, therapists, and support staff to deliver coordinated care
Support resident dignity and independence through person-centered care practices

Qualifications

Active LPN license (Licensed Practical Nurse) — New York state preferred
Completion of an accredited LPN training program
Current BLS/CPR certification or willingness to obtain upon hire
Strong interpersonal skills and the ability to work well in a team
Prior long-term care or skilled nursing experience preferred

Education

Graduate of an accredited LPN program (diploma or certificate)

Knowledge & Skills

Practical knowledge of medication administration and documentation
Ability to assess basic patient conditions and escalate appropriately
Familiarity with clinical charting and maintaining accurate records
Good communication skills with residents, families, and clinical teams
Comfortable following policies, safety protocols, and infection control practices
