Candidate Pay Rate: $32 per hour

Roles & Responsibilities

Provide compassionate direct patient care and support daily living activities
Administer medications and treatments safely and according to plan
Accurately document patient assessments, interventions, and responses
Communicate changes in patient condition to the care team and providers
Collaborate with nurses, CNAs, and interdisciplinary staff to deliver quality care
Support a culture of dignity and respect for residents and their families

Qualifications

Active LPN license for New York or eligibility to obtain one
Current BLS or CPR certification
Previous experience in long term care or a related setting preferred
Ability to pass background checks and required health screenings
Strong interpersonal skills and a patient-centered attitude

Education

LPN diploma or completion of an accredited practical nursing program
High school diploma or GED

Knowledge & Skills

Patient assessment and accurate vital sign measurement
Medication administration and documentation
Basic wound care and infection control practices
Electronic charting and clear clinical documentation
Effective communication and team collaboration
Time management and prioritization in a busy care setting
