Roles & Responsibilities

Provide compassionate, skilled nursing care to assigned patients
Administer medications and treatments according to care plans
Monitor patient condition and report changes to the care team
Document care accurately and communicate effectively with colleagues
Support patient comfort, safety, and dignity during every shift
Collaborate with RNs and providers to deliver coordinated care

Qualifications

Active Licensed Practical Nurse (LPN) license required
Basic Life Support (BLS) certification preferred or obtained upon hire
Clinical experience in a hospital or long-term care setting is a plus
Strong communication skills and ability to work as part of a team
Reliable, professional, and committed to patient-centered care

Education

LPN diploma or certificate from an accredited program
High school diploma or equivalent (required)

Knowledge & Skills

Medication administration and basic clinical assessments
Wound care basics and documentation best practices
Ability to prioritize tasks and manage time during busy shifts
Comfortable working with electronic health records
Patient safety, infection control, and standard precautions
