Roles & Responsibilities

Provide compassionate social work support to patients and families navigating mental health challenges
Conduct thorough assessments and coordinate care plans with the healthcare team
Support patients through the 5150 evaluation process with professionalism and empathy
Collaborate with interdisciplinary teams to ensure comprehensive patient care
Document patient interactions and care coordination activities accurately

Qualifications

Active California LCSW license required
Current BLS certification
5150 certification in California required
Prior experience with Dignity Health facilities strongly preferred
Two supervisory references required at time of submittal

Knowledge & Skills

Expertise in involuntary psychiatric evaluation processes
Strong understanding of mental health crisis intervention
Excellent communication and interpersonal skills
Ability to work collaboratively within a care coordination model
Knowledge of California mental health regulations and protocols
