Roles & Responsibilities

• Provide compassionate direct care to residents and support their daily living needs
• Administer medications and treatments accurately and safely according to facility policies
• Monitor resident status, recognize changes, and communicate effectively with the care team
• Document resident care and interventions clearly and timely in the charting system
• Collaborate with RNs, therapists, and interdisciplinary staff to deliver high-quality care
• Support a positive resident experience by promoting dignity, respect, and comfort

Qualifications

• Current, active LPN license in the state of practice or compact licensure as applicable
• Basic Life Support (BLS) or CPR certification preferred or required by facility
• Comfort working evening shifts and every other weekend as required by the schedule
• Previous experience in long-term care or skilled nursing is a strong plus
• Strong communication skills and a team-first attitude

Education

• Completed accredited Practical Nursing program or equivalent diploma

Knowledge & Skills

• Proficient in medication administration and accurate documentation
• Skilled in resident assessment, vital sign monitoring, and basic clinical procedures
• Knowledgeable about infection control and patient safety practices
• Able to use electronic health record systems and complete timely charting
• Excellent interpersonal skills for working with residents, families, and staff
