Roles & Responsibilities
- Provide compassionate assistance with activities of daily living, promoting dignity and independence
- Monitor and record vital signs and patient responses accurately
- Support safe mobility, transfers and falls prevention under established protocols
- Assist the interdisciplinary team with care plans and timely documentation
- Maintain a clean, safe environment and observe infection control practices
- Build rapport with participants and family members through clear, respectful communication
- Participate in trainings, fit testing, and required health screenings to ensure readiness for shifts
Qualifications
- Active CNA certification or equivalent (state requirements will be verified)
- BLS certification (required)
- License/Certificate Verification completed prior to assignment
- Ability to meet Trinity Health personal health requirements (vaccines, TB screening, drug screen, fit test, etc.)
- Comfort working in a person-centered care model with older adults
- Reliable, professional, and able to follow care plans and facility procedures
Education
- High school diploma or GED preferred
- State CNA certification or completion of an approved CNA training program
Knowledge & Skills
- Strong basic clinical skills: ADLs, vital signs, transfers and infection control
- Clear, compassionate communication with participants, families, and the care team
- Accurate documentation and attention to detail
- Ability to follow care plans and escalate concerns appropriately
- Teamwork and flexibility in a dynamic care environment
- Respect for privacy, dignity and cultural differences
