Roles & Responsibilities
- Provide compassionate care coordination for patients in need of psychiatric support
- Conduct thorough assessments and facilitate 5150 involuntary evaluations when appropriate
- Collaborate with interdisciplinary teams to develop comprehensive care plans
- Support patients and families through mental health crises with empathy and expertise
- Ensure compliance with all California regulations and facility protocols
Qualifications
- Active California LCSW license required
- Current BLS certification
- 5150 certification in California required
- Prior experience with Dignity Health facilities strongly preferred
- Strong clinical assessment and crisis intervention skills
Knowledge & Skills
- Expertise in involuntary psychiatric holds and 72-hour evaluation processes
- Knowledge of California mental health regulations
- Ability to work collaboratively within multidisciplinary teams
- Strong documentation and communication abilities
- Understanding of care coordination best practices
