Roles & Responsibilities
- Deliver personalized physical therapy services to patients recovering from surgery, falls, and other conditions in their homes
- Develop individualized care plans that help patients regain strength, mobility, and independence
- Collaborate with an interdisciplinary team to ensure seamless care coordination
- Document patient progress using HomeCare/HomeBase EMR
- Educate patients and families on safe exercise techniques and fall prevention strategies
Qualifications
- Current North Dakota Physical Therapy License
- BLS certification
- At least 1 year of physical therapy experience with previous home health experience preferred
- Valid driver's license and auto insurance
- Comfortable traveling within a 60-mile radius of Valley City
Knowledge & Skills
- Home health physical therapy expertise
- Strong assessment and care-planning abilities
- Familiarity with HomeCare/HomeBase EMR
- Excellent communication and patient education skills
- Ability to work independently in diverse home settings
