Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes throughout Tehama County
Conduct Start of Care visits especially for orthopedic patients when nursing is not ordered
Develop personalized treatment plans for orthopedic joint replacements and geriatric patients with multiple comorbidities
Complete approximately 5 meaningful patient visits per day using your clinical expertise
Document care efficiently using the HomeCare HomeBase EMR system on your provided tablet
Collaborate with the intake RN and manager to ensure seamless patient care coordination

Qualifications

Current Physical Therapy license in California
BLS certification required
Home Health experience preferred but will consider travelers with less than 2 years experience if they have Home Health background
Valid driver's license and reliable transportation for home visits

Knowledge & Skills

Physical Therapy expertise in home health settings
Experience with HomeCare HomeBase EMR system
Strong assessment and care planning skills for orthopedic and geriatric patients
Excellent communication and patient education abilities
Comfortable with independent practice and decision-making
