Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Conduct comprehensive Oasis assessments and start-of-care evaluations for home health patients
Develop and implement individualized physical therapy treatment plans focused on restoring mobility and function
Provide skilled interventions for elderly patients recovering from falls, fractures, and other orthopedic conditions
Collaborate with physicians, nurses, and other team members to ensure coordinated, high-quality care
Document patient progress accurately using a tablet-based system
Educate patients and caregivers on home exercise programs and fall prevention strategies

Qualifications

Active Physical Therapist license in Indiana (or compact eligibility)
Experience with Oasis assessments and start-of-care documentation preferred
Valid driver's license and reliable transportation for travel across four counties
Minimum of 1 year of home health or related experience is a plus

Knowledge & Skills

Strong clinical assessment and critical thinking skills
Excellent communication and patient teaching abilities
Ability to work independently and manage a daily caseload of 4-6 visits
Comfortable using technology (tablet) for documentation and scheduling
