Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:30 AM - 5:00 PM; occasional Saturday rotations once per month

Hours per Week: 40

Candidate Pay Rate: $58 per hour

Roles & Responsibilities

Assess and evaluate patients' physical therapy needs in a home health setting
Develop and implement personalized care plans to improve mobility and function
Educate patients and families on therapeutic exercises and equipment use
Collaborate with physicians and healthcare team to optimize patient outcomes
Document progress and adjust treatments to achieve goals

Qualifications

Minimum 2 years of experience as a Physical Therapist in acute, home care, or rehab settings
Current BLS certification required
Valid Physical Therapy license in California
Strong communication and organizational skills

Knowledge & Skills

Expertise in patient evaluation and care plan development
Proficiency in home health patient assessment and treatment
Knowledge of Medicare, Title 22, and regulatory guidelines
Ability to work independently and manage a caseload across multiple territories
