Candidate Pay Rate: $74 per hour

Roles & Responsibilities

Coordinate care for patients with mental health needs, ensuring seamless transitions
Conduct 5150 evaluations and coordinate involuntary holds when necessary
Collaborate with interdisciplinary team to develop and implement care plans
Provide crisis intervention and support to patients and families
Maintain accurate documentation in compliance with regulatory standards
Advocate for patient needs and connect them with community resources

Qualifications

Must have prior CA Dignity experience or you will not be considered
Active LCSW license in California
Current BLS certification
5150 certification in California (must complete training and pass exam with 80%)
Candidates may not have been directly employed by CommonSpirit facilities within the past year

Education

Master's degree in Social Work

Knowledge & Skills

Expertise in care coordination and discharge planning
Knowledge of 5150 process and involuntary psychiatric holds
Strong communication and interpersonal skills
Ability to work collaboratively in a fast-paced environment
Proficient in electronic medical records
