Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Conduct home health visits including start of care, recertifications, and resumption of care
Manage caseload of approximately 5-6 regular visits per day, or up to 3 visits on admission days
Complete OASIS assessments and document patient progress
Collaborate with clinical coordinators and sister agencies for support
Provide safety evaluations and occasional teaching visits for hospice patients

Qualifications

Current ND Physical Therapy License required
BLS certification required
Valid Driver's License and Auto Insurance required
Minimum 1 year of home health experience preferred
OASIS training preferred
Willingness to accept first-time travelers

Knowledge & Skills

Home health and OASIS experience
Excellent clinical assessment skills
Strong time management and organizational abilities
Ability to work independently in the field
Compassionate patient communication
