Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Conduct comprehensive OASIS assessments and start of care visits for home health patients
Develop and implement individualized physical therapy plans for patients recovering from falls, fractures, and other conditions
Collaborate with interdisciplinary team to ensure optimal patient outcomes
Educate patients and families on therapeutic exercises and fall prevention strategies
Document patient progress using electronic tablet system and manage approximately 4-6 patient visits daily

Qualifications

Current Physical Therapist license in Indiana
Experience with OASIS assessments and home health documentation
Compassionate and patient-focused approach
Ability to work independently and manage daily schedule efficiently

Knowledge & Skills

Knowledge of home health regulations and OASIS documentation standards
Strong clinical assessment and evaluation skills
Excellent communication and interpersonal skills for patient and family education
Time management and organizational skills to handle a variable visit schedule
Proficiency with tablet-based documentation systems
