Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with weekend rotation

Hours per Week: 40

Candidate Pay Rate: $46 per hour

Roles & Responsibilities

Manage a caseload of approximately 30 patients, coordinating their home health care
Perform start of care assessments, recertifications, and daily visits
Document patient progress using Homecare Homebase (HCHB) system
Collaborate with clinical managers and educators for support
Adhere to a point system to ensure productivity and quality care

Qualifications

Active Registered Nurse license in Kentucky (or compact state)
Minimum 1 year of MedSurg experience in a hospital setting
At least 2 years of Home Health experience required
Proficiency with Homecare Homebase (HCHB) is mandatory
Current BLS certification from AHA

Knowledge & Skills

Strong clinical assessment and critical thinking skills
Comfortable working independently in a home health setting
Excellent time management and documentation abilities
Familiarity with start of care, recertifications, and OASIS
Ability to build rapport with patients and families
