Roles & Responsibilities

• Monitor and amend patient visit schedules to maximize productivity and meet patient needs.
• Perform comprehensive OASIS and PT assessments of assigned patients, including psychosocial, educational, safety, and risk factors.
• Develop and implement patient-specific plans of care to promote optimal functional independence and safety.
• Provide skilled physical therapy treatments, including therapeutic techniques and patient education, assessing progress each visit.
• Coordinate patient care with physicians, nurses, caregivers, and other rehab services to ensure optimal outcomes.
• Supervise and direct clinical activities of PTAs, PT students, and home care aides.
• Complete timely and accurate documentation to support care provided and ensure billing compliance.
• Participate in in-service training and marketing of rehab services to the community.
• Perform patient health assessments and procedures as needed, such as PT/INR, wound care, and staple/suture removal.
• Collaborate with quality improvement staff to maximize accuracy of clinical notes and OASIS documents.

Qualifications

• Minimum 2 years of experience as a Physical Therapist
• Active Vermont state license or willingness to obtain upon acceptance
• Certification as a Physical Therapist following APTA guidelines
• Home health experience preferred
• Excellent communication and organizational skills

Education

• Graduate of an accredited Physical Therapy program
