Location: Rancho Cordova, California

Contract Type: Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM
Hours per Week: 40
Candidate Pay Rate: $49 per hour

Roles & Responsibilities
- Provide compassionate physical therapy services to patients across greater Sacramento county in their homes
- Treat a diverse caseload including cardio, pulmonary, orthopedic, neurological, and geriatric patients
- Help patients regain mobility and independence with personalized treatment plans
- Collaborate with interdisciplinary teams to deliver exceptional home health care
- Document patient progress and communicate effectively with supervisors and care teams
Qualifications
- Active Physical Therapy Assistant license in California
- Minimum 6 months of experience preferred, new grads considered in exceptional cases
- Strong clinical skills across multiple specialties
- Reliable transportation for home visits throughout the service area
Knowledge & Skills
- Home health experience
- Ability to manage a 5-6 patient daily caseload
- Strong documentation and time management skills
- Comfort working independently in various home settings
- Excellent communication and interpersonal abilities
