Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to 5-7 patients daily in their homes
Help patients recover from surgeries, falls, and other conditions with personalized care plans
Support both home health and hospice patients on their wellness journeys
Document care using HomeCare/HomeBase EMR system
Travel within a 60-mile radius of Valley City to deliver exceptional care where patients need it most

Qualifications

Current North Dakota Physical Therapy License required
BLS certification needed
1+ year of previous home health experience
Valid driver's license and auto insurance
Strong PT clinical skills with passion for home-based care

Knowledge & Skills

Physical Therapy expertise in home health settings
HomeCare/HomeBase EMR proficiency
Ability to work independently with minimal supervision
Excellent patient communication and care coordination skills
Comfortable traveling within assigned service area
