Roles & Responsibilities
- Provide compassionate physical therapy services across four rural counties
- Conduct Oasis Assessments and Start of Care evaluations for new patients
- Help elderly patients recover from falls and fractures with personalized care plans
- Manage a manageable caseload of 4-6 patient visits daily using a point system
- Document visits and track time efficiently with provided tablet technology
Qualifications
- Current Physical Therapist license in Indiana
- Experience with Oasis Assessments and Start of Care documentation
- Comfortable working independently in rural community settings
- Strong communication and time-management skills
