Candidate Pay Rate: $58 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes according to physician care plans
Assess and evaluate patients' therapeutic needs, muscle strength, mobility, gait, and rehabilitation potential
Develop and implement personalized treatment plans that help patients regain independence
Educate patients and families on therapeutic exercises, equipment use, and home safety modifications
Collaborate with physicians and care teams to report progress and adjust treatment goals
Supervise home health aides and coordinate with other healthcare professionals for comprehensive care
Document clinical progress and maintain accurate patient records to support quality outcomes

Qualifications

Minimum 2 years experience as a Physical Therapist in acute, home care, or rehab settings
Current BLS certification required
Strong oral and written communication skills with excellent organizational abilities
Valid Physical Therapy license for California practice
Ability to work independently while providing exceptional patient-centered care

Knowledge & Skills

Home health physical therapy assessment and treatment techniques
Patient evaluation for muscle strength, mobility, gait, and range of motion
Care plan development and goal setting for rehabilitation
Patient and family education on therapeutic exercises and equipment
Documentation and clinical reporting skills
Knowledge of Medicare, Medi-Cal, and regulatory compliance requirements
