Roles & Responsibilities

• Evaluate patients’ functional status and design individualized treatment plans to restore mobility and independence
• Implement skilled therapeutic interventions and progressive exercise programs to meet patient goals
• Document evaluations, plans of care, progress notes, and discharge summaries with clarity and timeliness
• Collaborate with nurses, OT, SLP, and interdisciplinary team members to coordinate comprehensive patient care
• Educate patients and families on home exercise programs and strategies to support recovery
• Participate in discharge planning to ensure safe transitions and continuity of care
• Contribute to a positive culture of safety, compliance, and patient-centered rehabilitation

Qualifications

• Active Physical Therapist license in Virginia (required)
• Doctor of Physical Therapy (DPT) or equivalent degree (DPT preferred)
• Open to new graduates — all experience levels, including new grads, are encouraged to apply
• Comfortable working in a skilled nursing facility (SNF) environment
• Strong documentation and interpersonal skills to connect with patients and the care team

Education

• Doctor of Physical Therapy (DPT) or graduation from an accredited physical therapy program

Knowledge & Skills

• Skilled in patient assessment and developing measurable goals
• Proficient with therapeutic exercise, mobility training, and safe transfer techniques
• Knowledge of documentation standards and regulatory requirements in post-acute care
• Excellent communication and teamwork to coordinate interdisciplinary care
• Ability to adapt treatment plans based on patient progress and tolerance
• Time management and organizational skills to balance caseload and documentation
