Candidate Pay Rate: $58 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes across Yolo County
Assess and evaluate patients' therapeutic needs, muscle strength, mobility, gait, and range of motion
Develop and implement personalized treatment plans in collaboration with physicians and care teams
Educate patients and families on therapeutic exercises, prosthetic device use, and home safety
Document patient progress and communicate changes in condition to physicians promptly
Supervise home health aides and coordinate with interdisciplinary team members
Identify additional home health service needs and make appropriate referrals

Qualifications

Minimum 2 years experience as a Physical Therapist in acute, home care, or rehab settings
Current BLS certification required
Strong oral and written communication skills with excellent organization abilities
Valid Physical Therapy license for California practice
Ability to work independently while thriving in a collaborative healthcare environment

Knowledge & Skills

Home environment evaluation and safety assessment
Therapeutic exercise prescription and gait training
Range of motion and muscle strength evaluation
Patient and family education techniques
Medicare and Medi-Cal compliance knowledge
Electronic documentation and clinical reporting
