Roles & Responsibilities
- Deliver personalized physical therapy services in patients' homes across Yolo County
- Assess and evaluate each patient's therapeutic needs and develop individualized care plans
- Guide patients and families through exercises, mobility training, and use of therapeutic equipment
- Monitor progress and communicate changes to physicians while maintaining detailed clinical documentation
- Collaborate with home health aides and coordinate additional support services as needed
- Participate in case conferences and contribute to ongoing team competency initiatives
Qualifications
- Minimum 2 years of physical therapy experience in acute, home care, or rehab settings
- Current BLS certification required
- Strong communication and organizational skills
- Valid PT license for California practice
- Ability to work independently while thriving as part of a collaborative care team
Knowledge & Skills
- Home health physical therapy assessment and treatment techniques
- Patient and caregiver education for therapeutic appliances and mobility devices
- Understanding of Medicare, Medi-Cal, and Title 22 regulations
- Clear documentation and clinical reporting practices
- Compassionate approach to supporting patients and families in home environments
