Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in accordance with care plans
• Administer medications and treatments safely and document accurately
• Assess patients' physical and emotional status, reporting changes to the team
• Collaborate with RNs, CNAs and interdisciplinary team members to coordinate care
• Maintain clean, safe environments and follow infection-control protocols
• Support patient and family education and encourage independence when possible
• Accurately complete required documentation and shift handoffs for continuity of care

Qualifications

• Valid Licensed Practical Nurse (LPN) license in New York (or compact/state equivalent as allowed)
• Current BLS (Basic Life Support) certification preferred or willingness to obtain
• Comfort working in long-term care or senior living settings preferred
• Up-to-date immunizations and ability to pass background and health screenings
• Reliable, compassionate, and able to communicate clearly with residents and team members

Education

• Graduate of an accredited LPN / vocational nursing program or equivalent

Knowledge & Skills

• Strong medication administration and documentation skills
• Basic assessment and wound-care knowledge
• Familiarity with infection control and standard precautions
• Good time management and prioritization in a busy care setting
• Teamwork and clear communication with interdisciplinary staff and families
• Comfortable learning facility-specific electronic charting systems
