Roles & Responsibilities

• Provide compassionate direct care to residents in a skilled nursing facility, focusing on dignity and comfort
• Perform nursing assessments, administer medications, and follow established care plans
• Collaborate with RNs, therapists, and care teams to ensure coordinated resident care
• Document resident condition and interventions accurately in the medical record
• Monitor and report changes in resident status promptly to the care team
• Support restorative care and assist with activities of daily living as needed
• Participate in shift handoffs and communicate clearly with colleagues and families

Qualifications

• Active Licensed Practical Nurse (LPN) license — NY license preferred
• Current BLS/CPR certification
• Experience or comfort working in a Skilled Nursing Facility (SNF) environment preferred
• Ability to pass background check and meet facility onboarding requirements
• Strong communication skills and a team-oriented attitude
• Current immunizations as required by facility policy

Education

• Completion of an accredited LPN/LVN program or equivalent vocational training

Knowledge & Skills

• Knowledge of basic nursing principles and medication administration
• Familiarity with SNF workflows, documentation, and care plans
• Comfortable with electronic medical records and charting
• Skilled at patient assessment, wound care basics, and vital sign monitoring
• Ability to prioritize tasks and handle multiple residents safely
