Roles & Responsibilities

• Provide compassionate, patient-centered nursing care for geriatric residents in a skilled nursing environment
• Assess resident needs, develop individualized care plans, implement interventions, and evaluate outcomes
• Administer medications and treatments accurately and safely
• Monitor clinical status, recognize changes, and escalate concerns to the care team
• Collaborate with physicians, therapists, social workers, and support staff to ensure coordinated care
• Document resident care clearly and timely in the electronic medical record
• Educate residents and families on condition management, care plans, and transitions
• Advocate for resident safety, dignity, and high-quality clinical outcomes

Qualifications

• Active Registered Nurse (RN) license in New York required
• Current BLS (Basic Life Support) certification required; ACLS or other specialty certifications a plus
• Experience in a Skilled Nursing Facility or geriatric care setting preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and teamwork abilities
• Ability to manage a varied caseload and prioritize resident needs

Education

• Associate Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred

Knowledge & Skills

• Clinical knowledge of geriatric nursing principles and common chronic conditions
• Medication administration and safe practice protocols
• Wound care and pressure injury prevention basics
• Infection control and prevention practices
• Familiarity with electronic medical records and clear documentation
• Effective patient and family education techniques
• Interdisciplinary collaboration and care coordination
