Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents, promoting dignity and comfort
- Administer medications and treatments safely and accurately following facility protocols
- Document resident care, observations, and changes in condition clearly and timely
- Collaborate with interdisciplinary teams to implement individualized care plans
- Monitor vital signs and report clinical changes to the RN and provider promptly
- Support admission, discharge, and transfer processes to ensure smooth transitions
- Assist in infection control, safety checks, and maintaining a clean care environment
- Mentor and support nursing assistants and new staff to uphold high standards of care
Qualifications
- Current Licensed Practical Nurse (LPN) license in New York State required
- Current CPR/BLS certification or willingness to obtain upon hire
- Previous experience in long-term care or rehabilitation nursing preferred
- Strong clinical assessment and medication administration skills
- Good communication and teamwork skills with residents, families, and staff
- Ability to follow care plans, chart accurately in electronic medical records, and prioritize tasks
Education
- Completion of an accredited Practical Nurse program or equivalent
- High school diploma or GED required
Knowledge & Skills
- Skilled in medication administration and basic wound care
- Comfortable performing resident assessments and reporting changes
- Familiarity with long-term care regulations and best practices
- Proficient with electronic charting systems and accurate documentation
- Excellent interpersonal skills for working with residents and families
- Strong time management and the ability to handle multiple priorities
