Roles & Responsibilities
- Assess and monitor residents' clinical status with compassion and attention to detail
- Administer medications and treatments safely and accurately according to orders
- Provide high-quality long term care including wound care, chronic disease management, and activities of daily living support
- Document assessments, interventions, and outcomes clearly in the medical record
- Collaborate with interdisciplinary teams and family members to create person-centered care plans
- Promote infection control, safety, and resident dignity at all times
- Support transitions of care and communicate changes effectively to the care team
Qualifications
- Current, unencumbered Registered Nurse (RN) license (state licensing requirements apply)
- Basic Life Support (BLS) certification required
- Demonstrated experience or interest in long term care (LTC) settings
- Ability to pass required health screenings and verifications
- Willingness to complete required immunizations and testing per facility policy
Education
- Graduate of an accredited nursing program (ADN or BSN preferred)
Knowledge & Skills
- Strong clinical assessment and critical thinking skills
- Familiarity with long term care best practices and resident-centered care
- Proficiency with medication administration and documentation
- Good communication and teamwork skills with patients, families, and staff
- Infection prevention and control knowledge
- Comfortable with basic EHR/charting workflows
