Roles & Responsibilities

• Assess and monitor residents' clinical status with compassion and attention to detail
• Administer medications and treatments safely and accurately according to orders
• Provide high-quality long term care including wound care, chronic disease management, and activities of daily living support
• Document assessments, interventions, and outcomes clearly in the medical record
• Collaborate with interdisciplinary teams and family members to create person-centered care plans
• Promote infection control, safety, and resident dignity at all times
• Support transitions of care and communicate changes effectively to the care team

Qualifications

• Current, unencumbered Registered Nurse (RN) license (state licensing requirements apply)
• Basic Life Support (BLS) certification required
• Demonstrated experience or interest in long term care (LTC) settings
• Ability to pass required health screenings and verifications
• Willingness to complete required immunizations and testing per facility policy

Education

• Graduate of an accredited nursing program (ADN or BSN preferred)

Knowledge & Skills

• Strong clinical assessment and critical thinking skills
• Familiarity with long term care best practices and resident-centered care
• Proficiency with medication administration and documentation
• Good communication and teamwork skills with patients, families, and staff
• Infection prevention and control knowledge
• Comfortable with basic EHR/charting workflows
