Shift Type: Per Diem Pool — flexible shifts as needed; schedule varies based on site needs (day, evening, night and weekend availability may be requested)

Roles & Responsibilities
- Provide compassionate, skilled nursing care to long-term care and hospice patients in the home setting
- Perform thorough patient assessments, document findings, and develop appropriate nursing interventions
- Administer medications and treatments safely and according to plan of care
- Collaborate with interdisciplinary team members to coordinate care and ensure continuity
- Educate patients and families about diagnoses, medications, and home care needs
- Complete required Trinity Skills Checklist and other competency validations as part of onboarding and ongoing practice
- Adhere to infection control, immunization, and safety protocols to protect patients and staff
- Maintain timely, accurate documentation and participate in quality improvement activities
Qualifications
- Active Registered Nurse (RN) license (Maryland) — required
- BLS (Basic Life Support) certification — required
- Ability to complete License/Certificate Verification and Trinity Skills Checklist
- Current immunizations and health screening per employer policy (PPD/IGRA, MMR, Hep B, Varicella, Tdap, COVID-19, Flu)
- Successful completion of Trinity 10 Panel Drug Screen and fit test per policy
- Experience in long-term care, home health, or hospice nursing preferred
- Strong communication skills and a patient-centered approach
Education
- Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) preferred
- Active RN licensure in Maryland required
Knowledge & Skills
- Clinical assessment and documentation in the long-term care/home health setting
- Familiarity with hospice principles and compassionate end-of-life care
- Medication administration and basic procedures in the home environment
- Infection control practices and immunization awareness
- Comfort with competency checklists (Trinity Skills Checklist) and skills validation
- Effective care coordination and interdisciplinary communication
